Applications are accepted Monday – Thursday 9 am to 3 pm at the address above. At the time of application, applicants must also provide:

• High school diploma and transcripts or GED and test scores
• Valid state-issued ID
• Birth certificate
• Social security card
• DD214 Military Report of Separation, if a veteran
• $10 application fee in cash only
